Node Feedback shows a form, in a block you can place on a node condition layout, to collect feedback from users for a given node type entry. That form runs on ajax so the user doesn't have to leave the page.
Each node has its own feedbacks list to review. There is also an admin report for all node feedbacks.

Install this module using the official Backdrop CMS instructions at https://backdropcms.org/guide/modules.

Visit the configuration page under Administration > Configuration > User Interface > Node Feedback (admin/config/user-interface/nf-settings) and enter the required information.

Add the Node Feedback form block to a node layout.

Setting permissions for the form: this module doesn't set any permission to show the form. You can do that whithin the block's configuration.

SPAM control: if you install the honeypot module you will see a new setting to add Node Feedback forms to honeypot's SPAM control system.
